This is general writing, not a plan
Nobody makes a referral for fun. By the time someone fills in a form they have usually been worrying for a while, and the thing they most want to know is what happens after they press send.

Support starts the same way whoever asks — a person, a relative, a social worker or a hospital discharge team. Nothing below commits anybody to anything.
- You talk to us. One call or one short form. There is no assessment paperwork to fill in before this.
- We visit. We come to you, meet the people who matter, and listen more than we talk.
- You get a written plan. Names, times and costs in writing before anything begins.
- Support begins. The same small team wherever it can be managed, reviewed with you rather than at you.

Less than most forms ask for. A name, a way to reach you, and one or two lines about what is going on. If a social worker or a hospital team is already involved it helps to know that, because it saves everybody repeating the same conversation. Everything else can wait for the visit — and nothing you say in a first conversation is a commitment.
If your enquiry is urgent, a form is the wrong route. The numbers that work today are on the contact page, at the top, above everything else on it.

This is where a lot of care websites become vague, so here is the honest position: the reply window is published and the start date is not. We answer enquiries within one working day. How quickly support can begin depends on where you are, what is needed and who else is involved, and a provider who quotes you a number before knowing those three things is guessing.

Sometimes the answer is that we are not the right fit, or not yet, or not for this. Saying so early is part of the job. When that happens we will say what we think would suit better, and we would rather have that conversation in the first week than in the third month.
